Please submit the info below to get started with therapy or consultation. I see therapy clients in person Monday-Wednesday 10am-4pm. Intensives and Consultation are scheduled on an individual basis. Once you send the info below including your insurance card, or agreement to cash pay, I will send you a link to the client portal with intake forms.
